Ambulance Plan 2009
Coinsurance Benefits and Policies

A. Each plan member shall be entitled to one year of emergency ambulance service to any hospital within the Pittsburgh area. Patient circumstances may be determine which facility is to be used. Service shall be provided as promptly as facilities and conditions permit. Should White Oak EMS be on another emergency or otherwise unavailable, for any reason, the responding ambulance may be from a mutual aid organization. The substitute ambulance shall be considered as being provided by White Oak EMS and transport shall be to McKeesport Hospital, our primary facility.

B. The "White Oak Plan" will attempt to limit out of pocket cost to our plan members.

C. Routine and pre-scheduled transports should be scheduled 24 hours in advance and shall be provided on an as available basis.

D. Membership:
$50 Household membership
$40 Individual membership
$45 Senior citizen's couple
$35 Senior citizen individual
RECENT CHANGES IN INSURANCE LAWS PROHIBIT SENIOR CITIZENS DISCOUNTS.
